How to Register
You can click the form linked above to register with the surgery.
Alternatively, please bring your NHS medical card to the practice and those of any other persons registering with you; if you do not have a medical card, you will be asked to complete a registration form for each person registering with the Practice.
In addition, all patients will be asked to complete a new patient questionnaire, allowing us to provide medical care in the interim period, while your medical records are transferred from your old practice to this one.
All new patients will be asked for a ‘new patient check’ appointment with the practice nurse.

In addition you may print off a registration form, fill it out and bring it in with you along with the medical questionnaire to the Practice; please also bring photographic ID with you.
Practice Area
Patients who wish to register should ideally live within the practice boundary, the practice may refuse an application to join its list if you do not reside in our area.
The Practice Boundary mainly covers Pendeford, Fordhouses, Oxley, Bushbury, Low Hill and Coven. Alternatively, the postcodes we cover are WV8, WV9, WV10 and part of WV11.

Catchment Area
Overseas Visitors
- Overseas visitors are exempt from charges for accessing primary care services. They are not entitled to free NHS hospital treatment. If hospital treatment is needed overseas visitors can contact- Overseas Visitor Team at Royal Wolverhampton NHS Trust at 01902695541 or email [email protected]
Temporary Registration
If you’re currently staying with a friend or family member within our GP surgery area for more than 24 hours but less than 3 months, you can ask to register as a temporary resident. You can find more information on registering as a temporary resident on the NHS website.
